The forms are schedules to the CIRP Regulations, published by IBBI — free, standard, and the same in every case. The amounts you state should be as on the insolvency commencement date.

Late claims may still be accepted within statutory limits, at the professional’s discretion and subject to the stage the process has reached — but a late claim risks the plan being finalised without you. File the moment you learn of the admission; do not wait for a perfect file of documents.
No fee is payable to the resolution professional or IBBI for filing a claim. Your only costs are your own — assembling proof, and counsel if you choose to engage one.
That depends on the outcome of the case and where your category ranks in the distribution waterfall — nobody can promise a number, and you should be wary of anyone who does. Yes — workmen and employees file on Form D (or through an authorised representative for a group). Workmen’s dues also rank relatively high in the liquidation waterfall, which matters if the case ends in liquidation.
